    Thanks!

    Spoiler:
That's pretty much along the lines I'm thinking, only I forgot the truck, and didn't think about a mask. The mask idea does work, and would explain the little girl's reaction to Sherlock. I also knew that Holmes figured it all out as the article suggests, and knew he didn't go to Molly for anything other than help in defeating Moriarty. It was all smoke and mirrors in the end, a show for Moriarty, and for Watson. But, like the writer of that article, I'm probably wrong, too.

I hope so. They were pretty hit-or-miss on that second season. When you have eliminated the impossible, whatever remains, however improbable, is ALWAYS A HALLUCINATION.

Was that the whole season? 3 episodes? How come I didn't know that?

Peter David's wife blogged her solution which included the idea that...

    Spoiler:
The rubber ball that Sherlock had throughout the episode would be relevant. There's an old magician's trick in which a person can hold a ball in their armpit to put pressure on a major artery and cut off the circulation to that arm - and, of course, Watson only had a chance to check Sherlock's wrist for a pulse.

"Hit or miss" strikes me as an odd phrase to use for a season of only three episodes. I think it's more accurate to say (referring to both seasons) that the Moffat episodes are brilliant, while the Gatiss episodes are not as much.
