Rescue Me is guaranteed to be quite upsetting at at least one or two moments every season but when Johnny Gavin (Dean Winters) died the Gavin's dad insists that everyone wears bright clothes to his funeral because "my boy was always laughing" I bawled like a small child.

Recently watched Summer Heights High and the last episode got to me.
I think it was season 3 of 24, where Jack Bauer is flying out to the middle of the desert to detonate a bomb and sacrifice himself. His last moments are on the phone with Kim Bauer. The "I love you, daddy" thing had my eyes well up. I think I'm a softy for parent-child relationships.
And LOST: Desmond and Penny in the "The Constant". God that was a great moment.

Charlie's death in LOST really got me too. Also the moment in The End where he remembered everything. Dominic Monaghan really gave a great performance on that show.
MASH. I know it's kind of retro, but my parents were big fans and we watched it religiously in reruns.
The farewell episodes were always the worst.
