Heeelp! Anyone got a 12 Step Program brochure?

For the THIRTEEN STEPS trade, Hester, Mellon, and I want to pattern the front matter after a 12 Step Program pamphlet. This is something we originally wanted to do with the credits pages of the single issues, but that didn't turn out exactly as we wanted.

Does anyone on here have access to a brochure and can give me a rundown of how it's structured, and maybe scan it so that the three of us can figure out how to design the title/credits pages, intro page, chapter breaks, and so forth?

Please don't fail us, Bendisboard. :)
